Introduction to Home-Based Respite Care
Home-based respite care HBRC is a crucial support system designed specifically for caregivers of patients receiving home palliative care, including those who have been Compassionately Discharged from acute hospitals. Its primary goal is to ease the burden of caregiving on these caregivers by providing temporary relief and assistance with their loved one's care needs.
How Does Home-Based Respite Care Help Caregivers?
HBRC facilitates respite by offering interim care provided by a professional caregiver sourced from a service provider who takes over responsibilities at home for a predefined duration. The services encompass:
Activities of dly living and personal care:
Bathing, including assistance with showering or bathing
Toilet management and hygiene mntenance
Nutrition and fluid intake support
Assistance in transferring the patient to bed or chr
Housekeeping tasks that promote cleanliness and safety:
Light cleaning like sweeping floors
Laundry using washing s
Mntenance activities to preserve the patient's physical and cognitive health, as prescribed by a healthcare professional:
Physiotherapy exercises
Simple wound care management
Higher-level care tasks for patients with complex needs:
Wound dressing
Stoma care
Tracheostomy suctioning
Avlability of Home-Based Respite Care:
Care is provided every day from Monday to Sunday, including public holidays.
Day shifts run from 8:00 AM to 8:00 PM
Night shifts operate from 10:00 PM to 8:00 AM.
For patients with more intricate care needs, the option for double dly shifts can be chosen. Each home palliative patient may use up to 30 single shifts lifetime, and Compassionate Discharge patients are entitled to two weeks consecutive days of support through HBRC. Singapore citizens and permanent residents enjoy government subsidies for these services.
Eligibility Criteria:
The applicant must be a caregiver who needs respite care, while caring for:
A patient currently receiving home palliative care from a MOH-subvented provider
Or a Compassionate Discharge patient from an acute hospital
How to Apply for Home-Based Respite Care:
Caregivers can apply through their home palliative care providers or the acute hospitals. The application process includes necessary clinical and psychosocial assessments conducted by these organizations.
